Fitness Assessment: How Fit Are You?


How physically fit are you? Would you like to step up to the next level of fitness, and enjoy the rewards of a healthier lifestyle and a better sense of well-being? Begin by filling out the Fitness Assessment to see where you stand now.

Once you've finished each section, click on the "Next" button. After completing the entire questionnaire for an overall assessment (try not to skip any sections!), click the "Finish" button.

Be Careful!

Before completing the rest of the fitness assessment, please answer these questions:
1.  Are you pregnant?
 Yes   No

2.  Do you have, or have you ever had, a heart or lung problem or disease of any kind?
 Yes   No

3.  Do you have or ever had high blood pressure, asthma or any other breathing problems, arthritis, diabetes, low back pain, and/or osteoporosis (or high risk for osteoporosis)?
 Yes   No

4.  Do you have dizziness that causes you to lose your balance, fainting spells, or any other troublesome symptoms?
 Yes   No

5.  Are you over 50 and not used to physical exercise?
 Yes   No

6.  Are you under a doctor's care for any medical conditions?
 Yes   No

7.  Are you taking any medications that may interfere with your ability to exercise?
 Yes   No

8.  Do you know of any reasons why it would be unwise for you to take part in physical exercise?
 Yes   No
